Items Requiring Custom Imprinting

For items requiring custom imprinting, please include the following information, either typed or hand printed: name, specialty, address, city/state/zip, telephone number and state license number (when required).

 

Special Printing

Don't see what you are looking for? Submit a sample or provide specifications for any folder, divider, patient registration form, clinical form, health history questionnaire, etc., and we will promptly provide you with a quote. We can print single or multiple part forms, custom imprint folders, and manufacture a wide range of chart dividers.

Please Note: All custom printed orders are subject to an industry standard over-run or under-run variance of 10%.

Free Shipping

All orders prepaid by Visa, MasterCard or American Express totaling $300.00 or more will be shipped surface freight free within the continental U.S. This offer excludes file cabinets, file storage, office accessories, office furnishings and special order items. We regret that the high cost of shipping outside the 48 contiguous states prohibits us from extending this service; we will use the most economical shipping method available to your location.

Shipping Policy

Most orders for in-stock items received by 2:00 PM (PT) are normally shipped the same day (excluding cabinets, file storage, office accessories and furnishings). Out-of-stock items are automatically backordered. Personalized stationery items normally ship within 5-7 working days, after proof approval. Custom printed orders normally leave our plant within 7-10 working days, after proof approval. If certain items require an extended ship time, such as those listed above, delivery time will be quoted when order is received.

Custom printing or personalization on a rush basis is available on most products for an additional charge.

Some combined stock and custom printed items can be shipped together if requested in advance. All orders are shipped via the most economical, expeditious method to your locale. Items ordered together may be shipped from different locations or may not be received at the same time.

Common carriers are used for large volume orders. Overnight, 2nd and 3rd day delivery services are available upon request.

Please Note: Shipments to California and Texas are subject to sales tax.

The shipping charges shown on your web order confirmation are an ESTIMATE. Actual shipping charges may vary from this estimate and will be charged on your final invoice.

This web site assumes that the shipping method you selected can deliver to the address you provided. If not, your order will be shipped using an appropriate available method, and the actual shipping charges may vary considerably from the estimated charges.

Orders are not invoiced until they are shipped. If you have entered credit card information, your credit card will not be charged until your order is shipped.

Terms & Conditions

Full payment is due upon receipt of merchandise. Accounts are considered overdue after 30 days and will be subject to a 1% monthly service charge. A service charge of $10.00 will be applied to all returned checks. For information regarding special financial arrangements, please contact our Credit Department at (800) 242-2376. For orders submitted from outside the U.S., pre-payment in U.S. Dollars (including estimated freight charges) is required.

Guarantee!

We guarantee our stock products. If you wish to return something, call our Customer Service Department at (800) 242-2376 to obtain a return authorization number and return shipment instructions.

Most of our stock items can be returned within 60 days of purchase for full credit, exchange or a refund of your purchase price, with prior return authorization. After 60 days, your return is subject to a 20% restocking charge.

Dymo® Products can be returned within 30 days of receipt with prior return authorization. Opened or unpackaged items are subject to a 15% or $15.00 restocking charge, whichever is greater.

For disposable products located in our Clinical Supplies section, opened packages are subject to a 15% restocking charge with prior return authorization.

Personalized items, custom printed items, custom manufactured or special order items, file cabinets, high density filing systems, and opened and unlocked software are not returnable.

Items damaged in transit will be replaced as quickly as possible. Note any visible damage to the cartons on the bill of lading or delivery receipt, and keep the original packaging in case items within are damaged.

Please Note: Self-adhesive products have a limited shelf life. Store in a cool, dry place.
